How I can apply for Home care services?

I have insurance and I'm not sure if it covers home health care services, will Param Healthcare Agency Homecare verify coverage prior to starting service?

Yes. Param Homecare agency dealing with Insurance company every day. We usually cover all HMO’s and we can check the eligibility using the system and let client know about the coverage prior to starting care.

If I don’t have Insurance still can I get the services as Private pay?

Yes, you can get services as Private pay client. Please contact to us and we will provide you all the detail.

In which county you provide services?

We have been providing our services to almost all the municipalities of Middlesex and to Somerset, Union, Morris, Passaic and Hudson and Bergen counties.

Do I need a physician's authorization for your services?

Yes, you need to provide prescription for Param homecare services. You can call to your doctor and get the prescription before starting the care.

Is my home care services cover by any kind of insurance or by my Medicare?

PHIS are Medicaid approved Home Care agency and we are under CAHC Accredited. We work under Medicaid guidelines and follow Div. of consumer affairs rules and regulation.

Medicare only provide home care in certain condition, if the client require skill nursing, continues to meet those criteria. Home care services are provided by the Medicare certified Home Health Agency and there is a limited for approval hours.

Is your Home Health aide insured or having background check?

Yes. Our home health aides are NJ Board of Nursing certified, well trained and highly experienced and are regularly assessed by our registered nurses, to maintain quality of the services provided. We have an excellent in-service program that helps our aides stay up to date with the current recommended guidelines for HHAs. For the safety and wellbeing of our clients, PHIS provide a criminal history background check clearance and cover each home health aide with general liability and workers’ compensation insurance. We have specific guidelines for the reference check procedures which is part of our policy, with strictly adhered to.

As a Client do I have a right to select my home health aide, who is available to provide me a care?

Yes. As a client PHIS always, encourage you in the involvement in the decision-making process. However, you must be satisfied. We always try to provide the care according to your choice. we are proud to say that we have bilingual aides who can provide services to multicultural population of the community.

How long will it take to get a Home care services?

As soon as your HMO authorization request received, based on your authorize Hours and needs, we can provide a caregiver immediately or at your convenience.

As a client am I authorize to pay Home health aide directly?

No. As a client you are not authorizing to pay our home health aide directly. (if you are not private pay client)

How is the privacy of my personal(Client) information maintained?

PHIS have a functioning process in place to protect, promote and respect of each clients. To comply with current federal and state health department regulations pertaining to the maintenance of client’s record. This start once our Nurse visit to client’s home for assessment and formal services agreement. All client’s Health records are confidential and maintained separately in locked room. Client’s information only assesses by RN, office personnel, who are directly involved in your services.

Can you give care to client who is admitted in a facility (hospital, nursing home, or assisted living)?

No, we can only provide Home care services at patient home.

What is the processor if a caregiver is unable to show up?

In case of call out sick or employment termination situation, we have additional staff recruited to cover the client when needed in these situations.

Also, we provide the Certified Home Health Aide Training Program that is approved by the Board of Nursing in the State of New Jersey, because of which we have access to huge database of certified home health aides.

How can client or other family members reach after hours, in case if it emergency?

Both Client and family can reach the agency after hours by calling on us on-call phone number. Clients are given all information on agency during Initial RN Assessment. Details of on call number is given below,

Office Operation:        Monday – Friday 9:00am until 5:30pm

On-Call Hours:            5:30 pm until 9:00am Monday – Thursday

5:30 pm Friday until 9:00am Monday
